Surgical Procedure Deposit Policy
Reservations will not be complete until we receive this form signed and dated. Please complete within 48 hours of receiving it.
Deposit Amounts

| Specialty Services (TSVS) | $500 |
| Surgical/Anesthetic Procedures (spay, neuter, dental, lumpectomy, etc.) | $150 |
Deposit Terms
Deposits are not refundable and will be accepted in cash or by credit card (Visa, MasterCard, Discover, or American Express).
Rescheduling Policy
Clients may reschedule up to three times within 90 days, provided notice is given 24 hours prior to the scheduled procedure. Same-day cancellations result in forfeiture of the deposit.
90-Day Limit
If surgery is not performed or the deposit is not used for the scheduled services within 90 days, the deposit is forfeited.
Final Payment
Final payment is due at surgical discharge, with the deposit amount credited toward your final bill. We accept cash, credit cards, and CareCredit.
